About This Product

General Incorporated Association National Joint Action on Medical Safety Recommended Software Act-FLOW In October 2015, a medical accident investigation system was launched. Behavior analysis is necessary when a certain human action results in a result different from what was expected, and "ActFLOW" was developed as software to support this behavior analysis. "ActFLOW" uses "ImSAFER" analysis, which is based on a human behavior model and is recommended as an effective analysis method for case analysis in medical safety manager training trainings sponsored by national nursing associations and various organizations. This software allows you to practice methods efficiently and effectively on your computer screen. It has excellent operability and allows you to easily create time-series event-related diagrams, etc. by affixing sticky notes to mock paper. Below, we will introduce the "ActFLOW" screen according to each step of the ImSAFER analysis method.

■Step 1: Creating a time-series event relationship diagram

It is created to organize the events and actions that occurred leading up to the error event and to understand the facts. Players such as people involved in the case (e.g., patients, doctors, nurses, pharmacists, etc.) and devices and equipment (e.g., ventilators, infusion pumps, etc.) are arranged on the horizontal axis, and the vertical axis is the time axis. Create an event flow diagram of how things happened.

■Step 2: Extracting problems

While following each event from the beginning of the time-series event relationship diagram, find the problems that are occurring, and mark the ones that seem to be problems with an "x" symbol. This symbol is called the "exclusion symbol."

■Step 3: Search for underlying factors
